Built for this kind of work.
Maintain an hours record for healthcare work without including patient information. This healthcare timesheet uses a unit or assignment code and a shift category, so scheduled duties, additional assignments and transfers can be reviewed with their actual times. Record each work interval clearly, especially when a shift crosses midnight or changes unit. The template does not determine on-call compensation, rest requirements, staffing adequacy or professional compliance. It is not a clinical chart or patient handoff record. Keep all clinical details in approved systems and use only the identifiers needed for authorized payroll or administrative review. Review breaks and exceptions according to the relevant workplace process.
What’s in the template
The blank form includes 14 entry rows and space to identify the person or team, reporting period and reviewer. Add rows in the editable files when your reporting period needs more detail.
How to use it
- Enter the worker and reporting dates.
- Use a unit or assignment code, not patient details.
- Record dated work intervals and reviewed unpaid breaks.
- Have on-call and other exceptions reviewed separately.
Keep the record useful
- Never enter patient information in this template.
- Separate on-call availability from actual work where required.
- Keep clinical handoff notes in approved systems.
These are recordkeeping tools, not payroll or legal advice. Confirm your own break, overtime, approval and billing rules. Do not include patient details, children's personal information or other sensitive information in general work notes.
Using it in Google Sheets
Download the Excel file, upload it to Google Drive, then open it with Google Sheets. Check the imported formulas and time formats with a sample entry before relying on totals. For shared teams, set access deliberately and keep a clean master copy.
