Built for this kind of work.
Create a clear personal work record for an employee whose hours need review by a supervisor or administrator. This employee timesheet keeps dated shift details together with a department or assignment field, so a reviewer can understand where the time belongs. It is intended for one person per sheet, not a whole team. Enter actual worked intervals and record unpaid breaks explicitly. Scheduled hours, leave balances, deductions and payroll calculations belong in the appropriate separate records unless your organization has instructed otherwise. Before submission, compare the entries with your own notes and resolve missing dates or duplicate rows while the work is still easy to remember.
What’s in the template
The blank form includes 14 entry rows and space to identify the person or team, reporting period and reviewer. Add rows in the editable files when your reporting period needs more detail.
How to use it
- Identify the employee and reporting period.
- Add the department or assignment for each shift.
- Enter actual shift times and unpaid breaks.
- Review missing entries with the supervisor before submission.
Keep the record useful
- Use one sheet per employee.
- Do not replace actual hours with scheduled hours.
- Keep payroll deductions out of the hours column.
These are recordkeeping tools, not payroll or legal advice. Confirm your own break, overtime, approval and billing rules. Do not include patient details, children's personal information or other sensitive information in general work notes.
Using it in Google Sheets
Download the Excel file, upload it to Google Drive, then open it with Google Sheets. Check the imported formulas and time formats with a sample entry before relying on totals. For shared teams, set access deliberately and keep a clean master copy.
