Make this blank log fit your process

Rename, add, remove or reorder columns. Download the customized PDF, then print it for your team. Nothing entered here is saved or sent to our servers.

Columns

Use 2–14 columns. Keep names short so there is room to write. All pages use landscape A4; larger row counts continue onto additional pages.

Preview shows four blank rows. The downloaded PDF uses your selected row count and may wrap headings differently.

Keep a record that can be reviewed

Use a row per subcontractor, project and policy type. Keep the contract requirement and evidence reference together so a reviewer can distinguish a received certificate from a completed coverage review.

Before your team uses it

  • Record the contract version or clause used for the review.
  • List each missing endorsement explicitly instead of marking the whole file complete.
  • Record the renewal follow-up date separately from the policy expiry.
  • Link to the document location rather than copying sensitive policy documents into the log.

Adapt it to your process

A certificate of insurance summarizes information; it does not amend a policy or guarantee that a contract requirement is met. Review the actual policy and endorsements with the appropriate qualified reviewer. Dates in this log do not trigger reminders.

Using this log

Does a certificate mean the subcontractor is fully covered?

No. The certificate alone does not establish that the policy covers the work or satisfies your contract. Keep policy terms, exclusions and required endorsements subject to an appropriate review.

Can I add or remove columns before downloading?

Yes. Customize the blank form in your browser, then download a PDF. Your edits are not saved after you leave the page. The PDF contains blank rows for your own records, not an online tracking workflow.
